Leave of Absence

 

If, for reasons beyond your control, you must discontinue your studies toward your PhD program for more than one semester, you must complete the Leave of Absence form to obtain prior written approval form your faculty advisor and the Graduate Coordinator.  You will be required to Reapply for Admission upon your return.
